Mmmmmm... so many *emphatics*, so little time. Lets not forget the humble underline, which back in the day of typewriters was an accepted norm, because you could push the carriage back and add underlining without requiring additional keys. The asterisks found use primarily for footnoting.

I prefer italics for sarcasm, you know, a little extra something to push the intent across.
For sarcasm that's explicit, I usually use :phones: or if its really raw, I use :evil:

 

Most of the time though, I don't indicate when I'm being sarcastic because I don't want to tip off deservedly clueless targets... :evil:

I find that it still goes over people's heads, though.

When Buffy *eschews*, I find it best to simply say gesundheit, and offer her a tissue. :phones:

But don't be surprised if I start singing Canned Heat songs (okay I'll explain that one: copy and paste "gesundheit" into http://www.thesaurus.com...)

Admittedly not dazzling, but it helps prevent the spread of dis-ease. :evil:
